What is Social Prescribing Service?
A Social Prescriber will work with you to help support and signpost you to local community services to improve your health and wellbeing. The service is for adults over the age of 18 who reside in Tower Hamlets. Community Services you could be signposted to: • Housing, benefits and financial support and advice • Employment, training and volunteering • Education and learning • Healthy lifestyles and active lives • Arts, music, outdoors and creativity • Befriending, counselling and other support groups What will happen in the consultation? Once a referral is made, the Social Prescriber will make contact with you via telephone to arrange a face to face consultation. During the consultation, you will be asked to elaborate on your reason for referral. Together, you and your Social Prescriber will explore different community services which are specialised in tackling your current issue. Contact will then be made to selected local community service and a referral will be processed. How can I be referred?
